Thank you for your interest in Tri-County Humane Society's adoptable animals! 

To ensure your adoption is in the best interest of both you and your pet, we ask that you answer the following questions. Please only fill out this questionnaire if you currently have an appointment scheduled to view animals and note your appointment date and time in the “comments” field. This is designed to help limit your time in the shelter and help us practice physical distancing.

All TCHS adoptable pets are microchipped. Upon adoption, the information below, as well as your contact information above, needs to be registered with the microchip company.

After adoption it is important to keep your pet's microchip information updated if you move or your phone number changes.

Most shelter animals have unknown medical backgrounds. We strongly recommend you take your new pet for a complete veterinary exam within one week of adoption and provide further medical attention if needed.
